Case management for spinal cord injury
The Case Manager is a coordinator, facilitator and communication link with both statutory services and private services for care, equipment, therapies and much more. We are a proactive CM team and our Case managers will become involved in the discharge planning wherever possible. Their role is to ensure entitlements to services are fully explored, they have the correct level of care and equipment. They also ensure the client is discharged to the most appropriate interim accommodation. We also work with the multi disciplinary team to ensure the client has a smooth discharge and ‘top up’ funding is recommended appropriately.

What our case managers can help with
Client motivation is crucial in achieving their goals and we work closely with the family , therapist, care providers, equipment supplies etc to ensure that these are all introduced at appropriate times giving the client time to adjust to the life changes they have experienced as a result of such a catastrophic injury.
